Description:

The Nurse (Care Coordinator) position is located at with the Crow Service Unit at Pryor Health Station and is responsible for ensuring essential coordination and support of medical care for patients.

DIRECT HIRE AUTHORITY JOB OPPORTUNITY ANNOUNCEMENT: These positions are eligible to be filled through Office of Personnel Management's delegated Direct Hire Authority [DHA]. The Direct Hire Authority has been authorized by the Homeland Security Act of 2002 and Part 337, Subpart B, Title 5 of the Code of Federal Regulations [5 CFR]. If filled utilizing DHA, the following is applicable: all applicants who meet the basic qualification requirements will be forwarded to the Selecting Official for consideration. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-

MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level (GS-9) in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:

Promote self-management of chronic disease through helping patients to identify barriers to making healthful choices. Help patient identify barriers to adhere to the prescribed regimen, providing coaching and guidance on positive health behaviors and providing chronic disease management. Assess, plan, implement and evaluate the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory and emergency conditions in the primary care clinic. Provide chart review to identify patients at risk for increased morbidity and mortality and recommended interventions. Provide patient and family education to assist in understanding and accept implications of care, diagnosis and treatment.

Time In Grade: Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
