Description:
Duties Work closely with the Nurse Case Managers and the primary care team to ensure patient-centered, whole person concept within each encounter. Monitor Disease Management of the patient. Conduct population health and medical management program evaluation and feedback, interpreting data and making evidence-based recommendations. Identify and facilitate reporting of process improvements to the Clinical Practice Guideline Champions and executive level committees. Conduct Quality Improvement (QI)
